For our communtiy climbing night, you can sign up multiple people through this form (i.e. participant and family members). Please provide the information for the participant here and as "Attendee 1" on the next page. You will provide information on all other attendees on the following page.
You are required to have a completed Release of Liability with OAS and with Bend Rock Gym. You can sign upon arrival. Please ensure that if you are bringing a minor and are not the parent/guardian that the parent/guardian must sign prior to participation in the event. Contact us for more info.
